Below is a list of the 12 action plans developed during the May 2-3, 2011 Summit meeting. These plans will be worked on over the course of the 12-24 months. If you are interested in working on one of these action plans, please contact us.

 

 


Support adoption and implementation

1. Standards Matrix Drive high value behavioral health care in the patient centered health home through a standards matrix.
2. HIT Develop and articulate HIT specifications to support integrated care.
3. Comparative Effectiveness Research Explore outcome variation in large implementations of integrated care and empirically identify best practices.
4. Peer Network Develop and support peer mentoring for successful behavioral health integration programs.
5. Parity Use existing federal parity law to seek reimbursement for essential elements of integrated care management.
6. Financing Develop and evaluate scalable financial incentives to promote adoption of effective integrated care.
7. Business Case Articulate the business case for integrated behavioral health care management and develop a national communication strategy to advance integrated care in the context of health care homes.

Adaptation and extension

8. Children Extend principles of integrated care to children under 13 years of age.
9. Severe Mental Illness Adapt principles of collaborative care to meet the needs of people with SMI.

Training and workforce development

10. Public Health Psychiatry Train psychiatrists for population impact.
11. Training Training programs for team-based integrated care.

Communication and marketing

12. Consumer Engagement Empower and mobilize consumers for patient-centered integrated care in health homes and ACOs.
