9. Severe Mental Illness

Adapt principles of collaborative care to meet the needs of people with SMI

 

Brief Description

  1. Review the key components of evidence-based integrated care programs in primary care.
  2. Develop a matrix of adaptations and additional elements that are necessary to address the needs of the SMI (severely mentally ill) population.
    1. Crosswalk this with SAMHSA EBPs
  3. Develop a stakeholder process to broaden and refine this matrix.
  4. Develop principles and tools for local adaptation and improvement.

 

Opportunity (Why Now?)

  1. Poor quality and substantial variation in quality of care
  2. High cost
  3. Lack of access to effective medical care
  4. Poor integration with the rest of health care
  5. High morbidity/mortality of SMI
  6. Substantial increase in awareness and concern about ‘gap’ in morbidity and mortality in recent years
  7. Opportunities to improve care, including health care home and dually eligible initiatives
