ADHD - Adult
The Adult ADHD Self-Report Scale (ASRS v1.1) is a tool used to identify patients who may have attention-deficit/hyperactivity disorder. ASRS v1.1 (pdf)

Anxiety
The Generalized Anxiety Disorder subscale of the Patient Health Questionnaire (GAD-7) is a quick and easy tool to help identify patients with anxiety and monitor treatment response. The GAD-7 is available free for clinical use at http://www.phqscreeners.com/overview.aspx in a variety of languages.

Bipolar
The Mood Disorder Questionnaire (MDQ) is a self-report instrument provided free from SAMHSA. MDQ (pdf)

The Composite International Diagnostic Interview (CIDI) is a free tool for the assessment of mental disorders, including bipolar disorder. CIDI (pdf)

Depression
The Patient Health Questionnaire (PHQ-9) is a quick and easy tool commonly used by health care and mental health professionals to help identify patients with depression and monitor treatment response. The PHQ-9 is available free for clinical use at http://www.phqscreeners.com/overview.aspx in a variety of languages. Functioning
The Global Assessment of Functioning (GAF) Scale is a numeric scale (0 through 100) used by mental health clinicians and physicians to rate subjectively the social, occupational, and psychological functioning of adults. GAF (pdf)

Obsessive Compulsive Disorder
The Yale-Brown Obsessive Compulsive Scale (Y-BOCS) Symptom Checklist is used to rate symptom severity in patients with obsessive-compulsive disorder (OCD).

Post-Traumatic Stress Disorder (PTSD)

Psychiatric Symptoms
The Brief Psychiatric Rating Scale (BPRS) is a clinician-rated tool that can be used to measure a wide range of psychiatric symptoms. BPRS (pdf)

Substance Abuse
The AUDIT-C is a simple 3-question screen for hazardous or harmful drinking. Available for free from SAMHSA: AUDIT-C (pdf)

Also free from SAMHSA is the DAST (Drug Abuse Screen Test) - a 10-item, yes/no self-report instrument that has been condensed from the 28-item DAST and should take less than 8 minutes to complete. The DAST-10 was designed to provide a brief instrument for clinical screening and treatment evaluation and can be used with adults and older youth. DAST (pdf)
